Roles & Responsibilities
1. Review medication orders, clinical appropriateness and dosage adjustments, interchanges and therapy modifications as appropriate as per policy before dispensing for the individual patient.
2. Conduct appropriateness review for each medication order (Drug, dose, frequency and route of administration, Duplication, Allergies or sensitivities, Interactions, Variation from hospital criteria for use, Patient’s weight and contraindications).
3. Medication reconciliation.
4. Timely and accurate dispensing of prescriptions to inpatients and discharge patients.
5. Identify and resolve medication related problems with the prescriber before processing the medication order.
6. Appropriate labeling of medications to ensure proper patient identification and safe medication use
7. Follows and maintains established procedures concerning High Alert Medications, Control medications, disposal of hazardous waste and comply with hospital policies.
8. Counsels patients or carers on prescription to ensure that the patient understands all information required for safe and proper use of medication and storage at home.
9. Promotes rational, cost-effective drug therapy
10. Compounds medications according to Hospital guidelines and procedures.
11. Increases Pharmacy-Physician interaction by providing an immediate source of information as well as encouraging the use of the hospital formulary of the approved drugs
12. Provides medication information to nurses, patients and other healthcare professionals.
13. Participates in quality use of medicines projects, audits and dispensary based quality assurance activities.
14. Arranges and processes ward stock replenishment of medications.
15. Assists and participate in educational programs (departmental, medical, nursing, community)
16. Ensures the maintenance and proper operation of equipments and supplies in the work area.
17. Assists the Chief Pharmacist on special assignments.
18. Demonstrates ability to interpret an extensive variety of technical instructions and deal with several abstract variables.
19. Follows insurance company’s policies and procedures.
