Provide direct patient care in the home setting, including administering medications, wound care, and monitoring vital signs.
Collaborate with other healthcare professionals to develop and implement patient care plans.
Educate patients and their families on self-care techniques and promote healthy lifestyle choices.
Assess and document patient progress, symptoms, and any changes in condition.
Coordinate and communicate with the patient's primary care physician and other healthcare providers.
Ensure that all necessary medical equipment and supplies are available and in proper working order.
Adhere to all legal and ethical standards of nursing practice.
Maintain accurate and up-to-date patient records and documentation.
Provide emotional support and counseling to patients and their families.
Stay current on advancements in home healthcare and attend continuing education programs as required.
