- Provide direct patient care in the home setting, including administering medications, wound care, and monitoring vital signs.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Educate patients and their families on self-care techniques and promote healthy lifestyle choices.
- Assess and document patient progress, symptoms, and any changes in condition.
- Coordinate and communicate with the patient's primary care physician and other healthcare providers.
- Ensure that all necessary medical equipment and supplies are available and in proper working order.
- Adhere to all legal and ethical standards of nursing practice.
- Maintain accurate and up-to-date patient records and documentation.
- Provide emotional support and counseling to patients and their families.
- Stay current on advancements in home healthcare and attend continuing education programs as required.
