- Assess a patient's condition and individual needs.- Create a comprehensive treatment plan for patients, outlining specific therapeutic activities.- Develop both short and long-term treatment plans, which may include dietary and exercise recommendations.- Instruct patients on basic movements that enhance their daily functioning.- Demonstrate exercises aimed at reducing pain and improving mobility.- Evaluate a patient's home or workplace to identify potential modifications that align with their health needs.- Document patient progress and provide further recommendations based on assessment results.- Adjust treatment programs as required to optimize patient outcomes.- Prepare materials and gather equipment needed for treatment sessions.- Educate patients on compensatory strategies for lost motor skills.- Assist patients with activities of daily living and offer guidance.- Illustrate therapy techniques and exercises.- Maintain thorough and accurate records of all related services and procedures.- Ensure the completeness and accuracy of comprehensive patient records in adherence to record-keeping standards and to support fellow team members.- Submit patient documentation promptly.- Undertake other relevant duties as required.- Attend KPI (Key Performance Indicator) training sessions for improvement and quality enhancement.- Participate in audits when necessary.- Be prepared for additional tasks related to EMR (Electronic Medical Records) implementation, coordinated with the Rehab Team Leader.
